Coordinated Support for Independent Living and Employment
Embark Supported Employment works with Managed Care Organizations to provide person-centered services that support daily living, employment success, and long-term community integration.
Supporting Independence Through Coordinated Care
Managed Care is a health care delivery system designed to manage cost, quality, and access to services. Through Medicaid Managed Care, individuals receive health and support services through approved Managed Care Organizations (MCOs), which work in partnership with providers like Embark Supported Employment.
Services Available Through Managed Care
Daily Living Skills
We provide real-life skill training to help individuals succeed independently at home, work, and in the community. Support may include:
- Budgeting and managing finances
- Balancing a checkbook
- Laundry and household tasks
- Meal planning and preparation
- Bus training and transportation skills
- Other daily skills needed for independent living and employment
Housing Counseling
Embark assists individuals in identifying housing options that fit their needs and financial situation. Support includes:
- Identifying housing barriers
- Assistance with renting or home ownership options
- Ongoing follow-along services to support successful community living
- Support for landlords as needed to ensure stability
Transportation
Transportation support may be provided, with authorization, to help individuals get to and from work sites and approved activities safely and reliably.
Community Prevocational Services
A 7-week, person-centered, community-focused program designed for individuals who need additional preparation before entering the workforce. This program focuses on:
- Soft skills development
- Personal and professional skill building
- Worksite visits and job exploration
- Volunteer experiences
- Guest speakers from local businesses and professionals
Enclave Services
Local businesses partner with Embark to provide small group work opportunities supported by on-site job coaching. Enclave services may include:
- Janitorial services
- Mowing and yard cleanup
- Snow removal
- Retail support
- Production and assembly work
- Food service
Participants are paid by Embark and are not considered employees of the host business.
Community Integrated Support
Once an individual secures employment and DVR services are no longer in use, Embark can continue to provide long-term, on-the-job support for both the employee and employer. This may include:
- Supported Employment Outcome-Based (SEOB) services through Inclusa
- Long-term support through additional approved funding sources
