Our Approach to Supported Employment
At Embark Supported Employment, we help individuals with disabilities discover their strengths, build confidence, and find meaningful work in the community. Our comprehensive support services focus on real-world employment opportunities, skills training, and long-term success guided by collaboration, dignity, and inclusion.
Employment & Community Support Services
Division of Vocational Rehabilitation Services (DVR)
We work alongside DVR to support individuals through every stage of employment from career exploration and job preparation to placement and ongoing workplace success.
Managed Care & Community Support
Through partnerships with Managed Care Organizations, we offer individualized support that helps participants build daily living skills, maintain employment, and stay engaged within their communities.
Project SEARCH Program
Project SEARCH provides young adults with immersive, real-world work experience designed to build confidence, develop job skills, and prepare for competitive employment.
Eligibility for Services
To receive services from Embark Supported Employment, individuals must meet the following criteria:
- Be 14 years of age or older. Individuals under 18 must have a valid work permit if earning wages.
- Be able to participate safely in services and not require a higher level of care than Embark is able to provide.
- Have a referral from an approved agency that has a service agreement with Embark, or arrangements for private pay services.
- Provide current documentation, such as a report from a doctor or qualified professional, confirming the presence and type of disability.
- Have an interest in and a desire to participate in Embark’s employment and support services.
How to Get Referred
Start With Your Referral Source
Complete Eligibility Screening
Complete a Functional Screen through your local ADRC to see if you qualify for services. This is often the first step for many individuals.
Connect Through Your Program
If you’re enrolled in Family Care, talk with your Community Resource Coordinator about employment services with Embark.
If you participate in the IRIS program, contact your IRIS Consultant to explore employment support options.
Choose Embark as Your Provider
After completing the DVR intake process and identifying your employment goals, you may select Embark Supported Employment as your service provider.

Getting Started with Embark
Orientation & First Steps
- You’ll be assigned a case manager to help set goals and work assignments.
- You’ll meet with your team to discuss work goals and objectives.
- You’ll learn about your rights and responsibilities.
- Staff will observe and evaluate how you learn, perform tasks, and interact with others.
- Staff will help you understand your skills and identify areas where you may need support.
Individual Program Plan (Person-Centered Planning)
- Plans are developed in a person-centered way, focusing on your individual goals and preferences.
- You will meet at least twice a year with your support team and Embark staff to review progress (semi-annual or annual review).
- Your case manager will also meet with you monthly and at six-month reviews to:
- Review your progress
- Set new goals
- Address questions or concerns
- Update your Person-Centered Plan
